Transcript Request
To receive a copy of your Salem Academy transcript, send your request via mail or email to Kara Weimer, Registrar.
Be sure to include the following information:
- Current name and name during student years, if different
- Graduation year; or year of withdrawal and grade level
- Current address and phone number
- Location for the transcript to be sent (email, fax, or mailing address)
